Job Summary
The Nurse Case Manager (NCM) will work directly with the primary care provider to assist in the careful oversight of Chronic Care Management. The NCM will assist the patient and their family to develop patient centered Plan of Care outlining the patient problem list and patient goal list. The NCM will become the first line of contact for CCM patients and the AHMP office addressing and resolving needs as they arise. Requirements
- 3 years experience in Home Health or related field
- Ohio Nursing License
- Superior knowledge base in Chronic Disease State Management.
Full Time;
- 50% Office
- 50% Direct patient contact
