If you are a patient with AHMP, accessing your Health Information is easier than you might think. You are now able to view your Patient Health Information directly from your home computer!. This includes the following:
- Access Lab History
- Prescription History
- Diagnosis History
- Visit Summaries
- Request Appointments
- Send communications directly to your provider
To enroll in this service, please send a request by clicking this link, (You must include your email). Upon receiving the request, you will receive two emails from our office. The first email will include a temporary PIN followed by the second email with detailed instructions on how to setup and view your information.
